REQUIRED DOCUMENTS FOR CLAIMS

Here at the ECOWAS Brown Card, Ghana National Bureau, we ensure that third party claims are handled promptly and fairly. In the event of being involved in an accident, kindly make claims with the appropriate documents depending on the claim being made to help facilitate the claim process.

DAMAGE CLAIM REQUIREMENTS

1. Application for the claim (Letter of Claim)
2. Police Report
3. Estimate / Invoice of repairs of damaged vehicle or property
4. Photographs of Damaged vehicle or property

INJURY CLAIM REQUIREMENTS

1. Application for the claim (Letter of Claim)
2. Police Report
3. Medical Report
4. Medical Receipt (if any)
5. Birth Certificate
6. Identity Card
7. 2 Passport-sized photographs, plus photographs showing injuries.

DECEASED CLAIM REQUIREMENTS

1. Application for the claim (Letter of Claim)
2. Police Report
3. Death Certificate
4. Letters of Administration
5. Birth Certificates of all Dependants of the deceased
6. Affidavits of Instructions from the Head of family
7. Identity Cards of Beneficiaries
8. Marriage Certificate (if any)
9. 2 Passport-sized photographs of the Dependants ( Wife, Children, etc.)
